School Nurses

Our two school nurses, Tammy Heath and Alyssa Holzberger, serve the five school buildings in the Mauston School District. In addition to promoting wellness and safety, the nurses supervise the care of ill or injured students, supervise the distribution of medications taken at school, manage care and health planning for students with chronic and acute health conditions, conduct hearing and vision screenings, and ensure compliance with state immunization requirements.

The student information form that is updated at the beginning of every school year is an important tool. Please fill it out carefully, noting any special health conditions or medications needed. If there is a change in your child's physical condition during the school year, please notify the nurse. It is also important for you to list your work phone and the phone numbers of two people who can serve as emergency contacts in the event that you cannot be reached.

Medication to be administered to children in school must be accompanied by a Medication Request/Consent Form. Non-prescription medications require both the parent's signature. School personnel may administer topical relief such as antibiotic ointment, calamine, A & D ointment, and sting kill. Prescription medications require both the parent and the physician's signature. This includes inhalers, EpiPens, and glucagon. These forms are available in the school office and at the Mile Bluff Clinic. When sending medication to school with your child, please send it in the original container labeled with your child's name. Do not send loose pills in a plastic bag; we have no way to verify what they are unless they come in a labeled container.

Each year children in kindergarten and first grade are screened for possible hearing problems; and children in grades one, three, five, and seven are screened for possible vision problems. Parents are notified by letter when a further examination is necessary.

Parents are urged to take care of minor injuries or illnesses which occur at home. Please make sure your children have a good breakfast and investigate their complaints if they are feeling ill. We ask parents to notify the school office if their children have a communicable disease such as chicken pox. We can watch for similar symptoms in classmates and possibly prevent an epidemic.

If your child becomes ill at school, we will contact you. If we cannot reach you, we will use the emergency contact information you have provided. Please remember we cannot keep seriously ill children at school. It is the responsibility of parents to give us current phone numbers for emergency purposes.

Please let us know when work phone numbers are changed, phones are disconnected, or if the emergency contact changes.
