BESTflexsm Plan Enrollment for 2013

Please select the information below for materials.  If you have any questions, please contact Sue Goyette at (608) 847-5451 ext. 6684.  Enrollment forms and waivers are due back to Sue Goyette in the District Office no later than November 21, 2012.  This is for the 2013 plan year (January - December) and is NOT part of the enrollment for the health insurance high-deductible program.  This is ONLY for the optional, annual Health Expense Reimbursement program or Dependent Care reimbursement program under section 125 administration.  Please note that the Federal Government has imposed a $2500 limit on Health Care Flexible Spending Accounts for plans effective January 1, 2013.

This explains what the BESTflex Plan is and how it helps participants save money on eligible health and daycare expenses. It reviews the claims and reimbursement processes, provides the web address to download forms and how to contact Participant Services. This brochure also explains how to complete the BESTflex enrollment form.

403(b) Salary Reduction Agreement
For changes to your contribution amounts, please complete this form, and send to Sue Goyette at the District Office. Contribution changes will take effect after approval by MidAmerica. For questions, please contact Sue in the District Office.
403(b) Transaction Routing Form
This form must be used for and/or accompany all transfer requests, distribution requests, loan requests, etc. Please complete this form along with any forms from your plan vendor and submit all documents to MidAmerica for approval. Contact Sue Goyette if you have any further questions.

Midamerica 403(b) Employee Information

You will find a copy of our plan highlights, as well as a Salary Reduction Agreement, and the notice to employees regarding the new 403(b) changes imposed by the IRS effective 1-1-09. If you wish to make changes to your investment or funds with your current vendor, please contact your provider (WEA Trust, American Funds, T. Rowe Price, etc.) directly. For changes in your salary reduction, please complete the form on the MidAmerica site and send it to MidAmerica directly. For questions, please contact either MidAmerica or the payroll department.

Required Notice - Youth Suicide Prevention Resources
Suicide Prevention Training for All School Staff
This brief workshop for all adults who work or volunteer in schools teaches a simple and effective skill for suicide prevention:ACT. A = Acknowledge the signs and symptoms; C = show Care and Concern for the person; T = Tell someone who knows how to intervene (pupil services staff). In this 25-minute presentation, School Psychologist John Humphries from DPI discusses relevant data about youth suicide in Wisconsin, risk factors, early and late warning signs, and suggests methods for dealing with suicidal students including ACT. ACT is a central skill taught in an evidence-based youth suicide prevention program called SOS-Signs of Suicide. SOS has been shown to reduce suicidal acts in youth. It is also an important skill for adults to know and be able to apply.
